FOOTBALL TAS: 2025 Match Incident Reporting & Investigation Process

Football Tasmania has created a new process for reporting and managing incidents which may occur on or off the pitch. This has been designed to ensure a consistent approach for clubs and individuals to report incidents to Football Tasmania, including details of incidents. This will assist in investigating and managing such incidents to resolution.
Football Tasmania (FT) take all reports seriously, however, matters reported without contact information provided or with insufficient evidence may result in these not being able to be acted upon.
A Match Incident Report Form should be completed for any incidents which clubs wish Football Tasmania to follow up.
The form should also be completed by as many individuals as possible who witnessed the incident. A word document version is available for handwritten incident reports.
Football Tasmania – Match Incident Report Form
The Match Incident Report Form can be used for any incidences which includes, but is not limited to:
• Racial Allegations (Towards Players/Match Officials)
• Discrimination Allegations
• Off Field Incidences (Social Media Breaches, Code of Conduct Breaches)
• Threatening/Offensive Behaviours towards a Participant
Incident/Investigation Process (Timeframe)
• Incident report form submitted by club/participants
• Acknowledgement of incident (within 2 business days)
• Request for further information/evidence (within 5 business days)
• Investigation notice issued to the opposing party (within 5-7 business days – providing an adequate timeframe for right of reply)
• Investigation outcome provided to all parties (noting, the outcome of the investigation could be finalised with sanction, no sanction, or referred to tribunal for outcome) 10-15 business days from alleged incident or as soon as possible with consideration of the nature and complexity of the incident.
